New Delhi, Oct 26 A patient suffering from chronic migraine for nearly a decade has reported improvement in symptoms after undergoing endoscopic migraine surgery at a Delhi hospital, doctors said.

The surgery was performed by Dr Swaroop Singh Gambhir, senior plastic surgeon at Sir Ganga Ram Hospital, after the patient failed to achieve sustained relief through conventional medical treatments.

The patient had been experiencing 20-25 migraine episodes a month, significantly affecting daily activities and quality of life, the hospital said.

Following a detailed clinical evaluation, the patient underwent endoscopic nerve decompression targeting trigger points in the temporal and occipital regions, which were identified as contributing to the symptoms.

After the procedure, the patient reported around 50 per cent relief in the intensity and duration of headaches in the temporal region, along with no persistent pain at the back of the head. However, a feeling of heaviness remained, the hospital said.

The patient also reported a significant reduction in medication use, with continued postoperative monitoring required to assess recovery and the durability of the improvement.

“Migraine surgery is a highly specialised procedure for patients with debilitating migraine attacks not relieved by medication and who have specific trigger points. It aims to improve their quality of life,” Gambhir said.

Endoscopic migraine surgery involves using a thin instrument equipped with a camera to access targeted nerves through small incisions. The surgeon releases surrounding tissues that may be compressing or irritating the nerves.

The procedure is considered only for carefully selected patients after a detailed assessment of their headache history, previous treatment response and potential peripheral nerve trigger sites.

The hospital said the surgery was not a first-line treatment for migraine and was not recommended routinely for all patients, and added that outcomes may vary depending on individual clinical findings and the suitability of the patient for surgical intervention.
